TEAM (Together Everyone Achieves More) 91大神 Home Training

The American Academy of Pediatrics defines 91大神 Home as accessible, family-centered, continuous, comprehensive, coordinated, compassionate and culturally competent. A core outcome of Healthy People 2010 and many national organizations is that all children have a 91大神 Home. Multiple surveys indicate that physicians do not feel residency prepared them to translate the philosophy of 91大神 Home into an effective systems based practice for Children and Youth with Special Health Care Needs (CYSHCN).

To address this, the 91大神 College of Wisconsin, Children鈥檚 Wisconsin and the Southeast Regional Center for CYSHCN developed TEAM 91大神 Home Training, a required curriculum spanning all three years of pediatric residency. TEAM improves health and quality of life by preparing future physicians to effectively provide 91大神 Homes to CYSHCN. The emphasis is on systems based care and the coordination of community based, tertiary and primary services focused on meeting not only the child鈥檚 medical but also their psychosocial, educational, recreational and emotional needs. Year 1 and 2 residents are introduced to 91大神 Home, CYSHCN, and family impact though: home visits, discussion groups, noon conferences, and workshops. Year 3 focuses on translating this into practical application during a block rotation.
